Treatment of facial telangiectasias with a diode-pumped Nd:YAG laser at 532 nm
Facial telangiectasiasare a common cause of cosmeticconcern. Current treatmentmodalities present various untowardeffects and limits. The pulsed dye laser has been consideredthe gold standard in efficacyand safety; unfortunately itcauses postoperative intracutaneoushematomata, discouragingmany patients from undergoingthis treatment. Several other vascularlasers (argon, tunable dye,copper, krypton, etc.) are disadvantagedby the risk of hypopigmentedand atrophic scars.
MATERIALS AND METHODS:
Weassessed a recent powerful versionof the potassium titanyl phosphate(KTP) 532 nm laser, whichdelivers sufficient energy in singlepulses lasting 10–50 msec. Collateral damage isreduced while the heating of thevessel is slow enough to avoidexplosive photothermolysis withits associated purpura. Sixty sixpatients with facial telangiectasiaswere treated.
RESULTS: In 62/66 patients(93.9%) we achieved a 75–100%clearance of the lesions, whiletwo treatments were needed toreach an acceptable clearance inthe remaining 4/66 patients(6.1%). The eventual need formore sessions was well toleratedbecause the acceptable postoperativeappearance allowedpatients to continue normal businessand social activities betweentreatments. No permanent complicationsor undesired effectswere noted.
CONCLUSION: We conclude thatthis diode-pumped frequencydoubledNd:YAG laser is aneffective device for the treatmentof facial telangiectasias, with alow profile of undesired effectsthat can be well tolerated bypatients.
