Periodontal pockets treatment


Clinical Explanation: 
It is a set of inflammatory diseases affecting the periodontium,i.e.,the tissues that surround and support the teeth. Periodontitis involves progressive loss of the alveolar bone around the teeth, and if left untreated, can lead to the loosening and subsequent loss of teeth. Periodontitis is caused by microorganisms that adhere to and grow on the tooth's surfaces, along with an overly aggressive immune response against these microorganisms.

Operation procedure: 
(decontamination procedure and therapy treatment)


1.While the lasing time is different, the laser motion used for both the decontamination procedure and laser therapy is exactly the same. And we penetrate to the depth of the pocket for both procedures.


2.For the decontamination procedure, the goal is to reduce the number of bacteria in the pocket around every tooth in the mouth to avoid bacteremia during the cleaning phase.So it will take 5-8 minutes to decontaminate the pocket space of 28 teeth,depending on pocket depth.


3.For the perio therapy treatment phase, of course,the goal is to eliminate the diseased tissue in the sulcus of all of the teeth in the quadrant being treated.It spends approximately 30-60s per tooth,again depending on the depth of the pocket and the severity of the inflammation present. This procedure adds about 5-7 minutes to the periodontal therapy appointment.


4.Hold the dental handpiece with a pen-like grasp,and keep high-speed suction next to the fiber while the laser power is on.Starting with the lowest power setting for the procedure that you are performing.

5.Placing the laser fiber into the gingival pocket,aiming the fiber toward the
tissue.Adjusting the power setting to the degree of inflammation as you work.(less power for less inflammation;more power as needed).

6.Using 2*2 gauze to clean the fiber tip each time you come out of the pocket.


Result:
Results were considered excellent when there was complete resolution without any skin changes (no color or textural changes) and no recurrence of the lesion after follow-up.


Advantage:
Laser treatment is painless and bloodless,it allows removal of only the diseased
tissue;meanwhile,it provides simultaneous coagulation of blood vessels,providing an optimal environment for speedy healing.

Introduction and Operation procedure:
Endodontic therapy is a sequence of procedures aimed at eliminating pulp infection and
protecting the tooth from future microbial invasion.


It consists of the following procedures:
– enlargement of the root canal

– removal of infected tissue
– disinfection of the root canal
– dense final filling

Diode laser could be used to finish the disinfection of root canal.


The length of the root canal, obtained through the X-ray measuring picture, is transferred
exactly to the fiber-optical waveguide to ensure that the flexible 200um optical fiber reaches
the physiological apex.


The fiber is introduced into the root canal to the apex without activating the laser. Only then
is the laser activated and the fiber guided in an apical to coronal direction with rotary
movements and in contact with the root canal wall. The root canals are filled lege artis after
the third session at the least.

Oral Hemangiomas



Clinical Explanation: 
Infantile hemangiomas are benign vascular neoplasms that have a
characteristic clinical course marked by early proliferation and followed by spontaneous
involution. During the proliferative phase in the neonatal period or early infancy, rapidly
dividing endothelial cell proliferation is responsible for the enlargement of these lesions.

Operation procedure:
Treatment was carried out in an outpatient setting solely using a topical anesthetic
EMLA cream (lidocaine 2.5% and prilocaine 2.5%) for at least one hour under occlusion
before treatment. For intra-oral portions of the lesions, topical anesthesia was applied
before field-block local anesthesia injection, in a circular manner surrounding the lesion.

1.Cases were managed using CHEESE 7W with wavelength of 980nm,CW,1.5W-2W,
400um bare fiber, the spot size was 10mm. The laser spot was adjusted to include 1-
2mm of the normal skin or mucous membrane beside the edge of the lesion within the
active beam field to be sure that the outer edge of the lesion was radiated.

2.The laser was coupled to chilled air-cooling apparatus during radiation of skin lesions,
but for the oral portion of the lesion,air cooling was not used because the forced cold air
flow can causes breathing difficulty in some patients.Compression was done from inside
the oral cavity to the outside during irradiation of the outer cutaneous side and from the
outside to inside during irradiation of the mucous membrane portion of the lesion.

Result:
Results were considered excellent when there was complete resolution without any skin
changes (no color or textural changes) and no recurrence of the lesion after follow-up.

Advantage:
Laser photocoagulation has been successful in the majority of patients, with minimal
morbidity damage to the surrounding tissue. As the CHEESE dental diode laser is a
deep photocoagulating laser, one that targets tissue protein, epidermal changes - and
sometimes scarring - may result after treatment; thus, to minimize the cryogen cooling of
the skin, application should aim to protect the epidermis and papillary dermis while
achieving deep tissue photocoagulation during diode laser irradiation.
