Bleaching Treatment


Description:
Suitable bleaching materical absorb the energy of the laser and moderate increases the temperature to accelerate the chemical reaction process. Method:
The gel will be applied to all the teeth- if necessary apply also coffer dam for protection of the gum- and irradiated with bleaching handpiece in a distance of about 0.5-2.0 cm with 2 watts and approx with 15—25 seconds. The bleaching handpiece will treat about 4—5 teeth once, so you need to do it many times to complete all the teeth. Wait 1 minute after treating all the teeth first time then do it again for another three times.

The gel will be wiped off after treatment and the color is checked. Repreat this procedure untill the color is satisficed.

The bleaching treatment does leed to lasting hyper sensitivities but doctors should pay attenion to the feeling of patients.

Laser Tissue Interaction


Laser use in the field of medicine is large and steadily growing. This growth is based on the versatility of laser light. The characteristics of laser light are defined by its wavelength, pulsed or continuous wave operation as well as its average power. The unique capabilities to tailor laser light to a particular application opens up a wide space for laser tissue interaction. These interactions can be classified into three main categories:

Photothermal Reactions:
Laser light absorbed by chromophores in the tissue is converted into heat. A typical application is photocoagulation, where the laser light is absorbed by hemoglobin to stop bleeding or to seal blood vessels. Another example is thermal ablation when laser light vaporizes tissue water for tissue cutting. This interaction requires laser solutions that have high average power and a wavelength that matches the absorption levels of target tissue.

Photochemical Reactions:
Photons absorbed by tissue molecules. Excited molecules can undergo chemical reactions. A prominent example is Photodynamic therapy (PDT) where a photosensitive drug is administered. Using specific wavelengths enables applications such as selective photo thermolysis. Tattoo removal is an example of this. This interaction requires laser solutions that have high average power and a wavelength defined by molecule absorption.

Photoablation:
Laser light is used to break the molecular bonds in the tissue. Key applications include ophthalmology where UV laser light is used for refractive surgery of the cornea, as well as in lithotripsy where high energy laser pulses are used to generate plasma and shock waves that can break up kidney stones. Typical laser solutions operate in pulsed mode for high peak power and, depending on the type of tissue, have UV to NIR wavelengths.

Endovenous Laser Ablation of Varicose Veins with the 1470nm Diode Laser


Background
Endovenous laser ablation (EVLA) is one of the most accepted treatment options for varicose veins. In previous studies conducted with a laser at 810 to 1320 nm, paresthesia, pain, and ecchymosis were common adverse effects. We hypothesized that a lower linear endovenous energy density (LEED), as used with 1470-nm diode laser fibers, would lead to a reduction in adverse events.

Methods
We conducted a prospective, nonrandomized observational cohort study of 312 consecutively treated lower limbs legs in 286 patients. Of these, a bare laser fiber (ELVeS-plus kit) was used to treat 168 legs in 150 patients (group 1), and a radial fiber (ELVeS-radial kit) was used in 144 legs in 136 patients (group 2). Laser treatment was performed in the great saphenous vein. Follow-up for all patients was 3 months. The primary end point was the occurrence of ecchymosis and bruising. This was correlated to the reduced LEED needed with the 1470-nm diode laser.

Results
Laser fiber (odds ratio [OR], 22.3; 95% confidence interval [CI], 20.2-24.5) and body mass index (OR, 0.35; 95% CI, 0.15-0.55) were identified as independent parameters for LEED. In group 2 compared with group 1, LEED in the great saphenous vein could be reduced from 79.4 ± 9.1 to 57.4 ± 10 J/cm (P 

Conclusion
Endovenous laser treatment of varicose veins in the great saphenous vein with the 1470-nm diode laser is safe and highly effective. The lower energy level needed using the radial laser fiber significantly minimized adverse effects compared with the bare laser fiber.

Lasers in BPH Treatment


For the last few years, different types of laser and different wavelengths have been proposed as candidates to be considered real alternative to the TUR.

Diode Laser (980nm)
Preliminary results suggest that prostate vaporization using 980 nm diode laser with output power up to 120 W, is effective and associated to minimal morbidity. It enjoy following advantages:
1. Highest absortion in water and haemoglobin from the same wavelength
2. Selective light vaporization (SLV)-combines perfectly non-contact vaporization and coagulation
3. Ability to work in continuous and pulsed mode to allow efficent removal in all tissue types
4. Optical penetration depth of the 980 nm is higher than for the KTP or Ho: YAG lasers


Nd: YAG laser
(1064 nm wavelength) has a low coefficient of absorption in the tissue and achieves a deep tissue penetration; it produces a necrosis by deferred coagulation, without appreciating immediate ablative effects. It has ben given up in the treatment of the BPH due to Its morbidity and low efficiency.

Holmium: YAG laser
(2140 nm wavelength) has a high absorption in prostatic tissue, which carries a high density of energy and a minimal depth penetration, allowing the vaporization of the tissue. It has been used in the treatment of the BPH either by vaporizing the tissue (HoLap) or by performing an split of the adenoma and carry out the enucleation of the gland (HoLep) . The KTP laser

Introduction and Operation procedure:
The main technical obstacles in laser assisted DCR are to deliver a sufficiently powerful laser beam via a relatively narrow optical fiber, which in turn fits into an endo canalicular probe.
We will use 980nm diode laser in repetitive mode, Power 10Watt, Ton (Pulse length) 90ms, Toff (Pulse interval) 50ms. 
Osteotomy was achieved by applying laser energy via an optic fiber, which we inserted into the lacrimal sac with a probe. Irrigation with 0.9% saline solution of the endocanalicular probe was used to prevent overheating of the probe. The size of osteotomy was controlled through the nasal endoscope. Once an opening of at least 5 mm in diameter has been achieved, application of laser energy ceased.
