Diode Laser VS ND:YAG Laser for Spine Surgery

Percutaneous Laser Disc Decompression and Nucleotomy has been performed at moment worlwide on more than 60000 patients. The more used lasers were KTP 532nm, Ho:YAG 2100nm, Nd: YAG 1064nm. Their combined success rate (excellent/good to fair) according to Macnab and Oswestry score were more than 80%, with a complication rate of less than 1.5%(7,8,9). In order to obtain a good result it is very important not only a proper selection of the patients, but also a good choice of the laser used.

With regard the use of the Diode 980nm, we believe to be the best and more advanced laser in the treatment of disc herniation, related to the optimal water absorption. Because 980nm is 10 times more absorbent than 810nm and 5 times more absorbent than 1064nm, requiring less laser energy; this implies less heat diffusion in surrounding tissues and no undesirable side effects. A first introduction of Diode 940nm in disc herniation treatment has been performed in 1998 by Hellinger (10) in a prospective randomized study versus Nd: YAG 1064nm. The overall success rate (90%) confirm the proper use of Diode in order to decompress the nerve root by disc herniation. Nakai (11) also confirmed in an experimental study with a Diode 810nm, that Diode is less aggressive in the surrounding tissue, preserving the end plate and the vertebral body from any damage. No secondary changes on the intervertebral disc and adiacent vertebral body after Diode laser disc irradiation were detected.

Experimental studies performed both on human and specimen lumbar discs using the Diode laser 980nm showed an absorption of laser light of 90.27% in the disc and a retraction of about 55% (+/- 1.7) On 2.7mmq of the tissue after laser treatment

For these reasons the Authors believed using the specially designed and optimised Diode 980nm Laser
 in the treatment of disc herniation, confirming the overall success rate of the literature (7,8,9,13,14), without any complications related to heat diffusion in surrounding tissues (aseptic discitis and spondilytis, bladder injuries, vascular injuries to the abdominal cavity, abdomen injuries).

Plastic Surgery Diode Laser



The diode laser is a very intense laser that has the ability to both treat vascular problems and pigment problems which include; unsightly spider veins or “broken blood vessels” of the face, rosacea, and people with age or “liver” spots of the face, hands, neck, or chest “V.”

About the Procedure
The procedure is performed on an outpatient basis usually without anesthesia, although for very sensitive areas and younger patients it can be used after topical anesthesia.
The laser has a very fine target and the individual vessels or pigmented spots need to be traced with the laser light. This results in high numbers of rapidly fired pulses.

Recovery Time
There is minimal disturbance to the skin and rarely any recovery period, although swelling for a few hours or a day isn’t rare. Commonly, a touch-up procedure may be needed to treat the few areas that do not fully respond to the first treatment.

Pain Relief Laser



Why Laser is Different?                                                          Laser therapy is a medical breakthrough therapeutic device with unparalleled applications and treatment outcomes. The Laser works by converting light into biochemical energy, resulting in normal cell function, which causes symptoms (PAIN) to disappear. The primary biological action of laser results from stimulation of cellular transport mechanisms in the mitochondria, cell membranes and epithelial tissues. This action causes the release of vasodilating chemicals, the stimulation of DNA and RNA (Building blocks) synthesis, an increase in enzyme production, an increase of superoxide dismutase activity, normalization of tissue pH, and increased ATP production. The increase of vasodilatation (blood supply) and improved microcirculation will increase the supply of cellular nutrition promoting tissue repair and remodeling (which means you heal faster). Significant reduction in edema (swelling) is also noted.

What does laser therapy have over other forms of therapy?
 It does not require the use of drugs or surgery, there are less side effects or risks, and it is quick and convenient. Studies have shown that it is equal to or more effective than other forms of physical therapy. Pain relief is often immediate.

<strong><strong><strong> </strong></strong></strong>Does it hurt? What does the treatment feel like?
There is little or no sensation during treatment. Occasionally one feels mild, soothing warmth. There is no pain associated during laser application. Laser treatment is relaxing and some people even fall asleep. On the other hand, it sometimes occurs that pain can increase or temporarily worsen 12-24 hours after a teatment session. This is because the laser light starts the healing process. This type of pain is particularly likely to arise when the health problems are chronic in their nature. It usually subsides after a few days but can, in rare cases, last longer. This is a normal reaction if it occurs, and will subside.



How long does the treatment take?
The typical course of treatment is 10-minutes, depending on the size of the area being treated. Acute conditions may be treated daily, particularly if they are accompanied by significant pain. More chronic problems respond better when treatments are received 2 to 3 times a week. Treatment plans are determined on an individual basis.


How many treatments does it take?
This depends on the nature of the condition being treated. For some acute conditions, 2 to 6 treatments may be sufficient. More chronic conditions may require 6-10 sessions. How long before results are felt?

High Power Laser Therapy vs. Cold Laser Therapy

What is the Difference?

It’s All about the Power!
The single most important factor that determines the effectiveness of Laser Therapy is the power output (measured in milliwatts (mW)) of the Laser Therapy Unit. It is important for the following reasons:
     1.  Depth of Penetration: the higher the power, the deeper the penetration, allowing for treatment of tissue damage deep within the body.
     2.  Treatment Time: more power leads to shorter treatment times.
     3.  Therapeutic Effect:  the greater the power the more effective the laser is in treating more severe and painful conditions.
Different Classes of Laser Therapy
Although the Food and Drug Administration (FDA) and Health Canada recognize four different classifications of lasers (Class I – Class IV), only Low Level Class IIIb and High Level Class IV lasers have been demonstrated to provide health benefits to patients.
Low Level Laser Therapy (Class IIIb):
  •     Power Output = 25 - 500 mW
  •     Depth of Penetration: Less than 0.5 cm
Class IIIb Lasers have been in use treating a variety of healthcare conditions for many years and are defined as having a total power output of 25-500 mW. These lasers are commonly referred to as “Cold Laser” or “Low Level Laser Therapy (LLLT)”. Due to their extremely low power output, these “Cold Lasers” are only capable of penetrating up to 0.5 cm into the body and treatment times for simple conditions can be as much as 1-2 hours. These Cold Lasers are limited to conditions related to the skin or just below the skin, such as superficial ligaments and nerves in hands, feet, elbows and knees. Low Level Laser Therapy (LLLT) has virtually no impact on the treatment of joint, muscle, nerve, bone or spinal disc conditions since most of these body structures are located deep beneath the skin. Therefore, Cold Laser Therapy is often considered obsolete by Laser Therapy standards.

High Power Laser Therapy (Class IV):
  •     Power Output = 1 - 15,000 mW (30 times more powerful than Class IIIb Lasers)
  •     Depth of Penetration: > 4 cm
Class IV High Power Lasers are able to treat all the same conditions that respond well to Cold Laser Therapy as well as many more. Because High Power Lasers are able to penetrate more deeply into body tissues, the vast majority of muscles, ligaments, tendons, joints, nerves and skin can be treated effectively.
Treatment times are also dramatically reduced when treating with High Power Laser Therapy. As mentioned above, Low Power Lasers (25-500mW) often require 60-120 minutes in order to deliver a therapeutic dose to the affected body part. High Power Laser Therapy (500mW – 15,000mW), on the other hand, delivers an equivalent therapeutic dose in a fraction of the time. In fact, the 15W High Power Laser can deliver a therapeutic dose of laser energy to many injured parts of the body within 5-15 minutes!
In summary, High Power Laser Therapy can treat many more conditions in much less time with far better results than cold laser.

If Class III lasers are therapeutically ineffective, it is because of insufficient energy or dosage, combined with poor penetration. Class IV lasers offer better therapeutic outcome, based on six characteristics of this new technology:

1. Larger dosages of therapeutic energy. Class IV lasers can deliver up to 1,500 times more energy than Class III and consequently reduce treatment time.

2. Deeper penetration into the body. Leading Class III lasers only penetrate 0.5-2.0 cm2. Class IV can penetrate up to 10 cm2

3. Larger treatment surface area. Class III cover a treatment area of 0.3-5.0 cm2, depending on the model and manufacturer. Class IV cover up to 77 cm2. This is important when treating large regions, such as the lumbar spine, quadriceps or hips.
4. Greater power density. Power density indicates the degree of concentration of the power output. This property has been shown to play a major role in therapeutic outcomes

5. Continuous power supply. In Class III lasers, the power is pulsed or modulated approximately 50 percent of the time. In other words, light is permitted to pass through the probe for only 50 percent of the total operating time. 
6. In most cases, Class IV lasers deliver a consistent amount of energy over a given time. Their power can be adjusted for acute and chronic conditions.


7. Superior fiber optic cables. Fiber optic cables transmit laser energy from the laser to the treatment probe (wand) at the end of the cable. Several studies reveal that as much as 50 percent of the light energy generated by a Class III laser may be lost by the time it reaches the end of the probe.

Low Level Laser Therapy vs. High Power Laser Therapy (HPLT)




 Class IV High Power Laser is NOT a Class III, “cold laser” or a Low-Level Laser Therapy (LLLT/Low Intensity Laser Therapy/LILT, Cold Laser) device. It is a high-power, therapeutic laser with the ability to stimulate accelerated healing energy within a continuum of treatment depths from superficial to extremely deep (6-9 inches). This enables the accelerated treatment of superficial injuries, sprains/strains, and deep intra-articular tissues (inside of joints) unreachable by any other physical therapy modality, Class III “low-level“laser or even other Class IV lasers.

Varicose Veins —Endovenous Laser Therapy

What is EVLA?  
EVLA is a new method of treating varicose veins without surgery. Instead of tying and removing the abnormal veins, they are heated by a laser. The heat seal the walls of the veins and the body then naturally absorbs the dead tissue and the abnormal veins are destroyed. It can be carried out in a simple treatment room rather than an operating theatre.


Are my veins suitable for EVLA?
Almost all patients with varicose veins are suitable for EVLA. Those few who are not suitable (usually those with small recurrent veins after previous surgery) can usually be managed with just foam sclerotherapy.


What does the procedure involve?
An ultrasound scan is performed and the veins to be treated are marked with a pen. You lie on a couch and your leg is cleaned and covered with drapes. Depending on which veins are to be treated, you may be on your back or your front. All these steps are guided by ultrasound scanning.
Endovenous means inside the vein, so the next thing the doctor has to do is to get inside your vein. A small amount of local anaesthetic is injected into the skin over the vein and a needle inserted into it. A wire is passed through the needle and up the vein. The needle is removed and a catheter (thin plastic tubing) is passed over the wire, up the vein and the wire removed.
A laser fibre is passed up the catheter so its tip lies at the highest point to be heated (usually your groin crease).   A large quantity of local anaesthetic solution is then injected around the vein through multiple tiny needle pricks. All staff and the patient put on laser safety specs as a precaution.   The laser is then fired up and pulled down the vein over about 5 minutes. You will hear a warning buzzer ringing and may smell or taste burning but won’t feel any pain.   If you’re having both legs treated the process is repeated on the other leg. The laser and catheter are removed and the needle puncture covered with a small dressing.
The treatment takes about 20-30 minutes per leg.   You may also have some foam sclerotherapy or some avulsions undertaken and a compression stocking is then put on.
