LASER – ENT SURGERY


The laser techniques are widely used in ENT pathologies.

Thus, in the pathology of the ear, benign tumors of the pavilion of the ear and external auditive duct can be removed by laser vaporization. The serous otitis can be solved by a laser myringotomy intervention, which is the creation of a tympanum outlet, drainage and aspiration of the tympanum.

In the pathology of the larynx a great part of the benign tumors (vocal nodules, vocal cords polyps, laryngeal papillomatosis) and even the malignant tumors (generally, in an early stage) can be solved by endolaryngeal laser surgery.

The most widely used laser surgery procedures, with an optimum applicability in a private practice are those that target oropharyngeal pathology and nose pathology.

Chronic cryptic tonsillitis, so widely encountered in the medical practice and the therapeutic solution of which is too commonly thought to be tonsillectomy (removal of tonsils), can be treated by laser surgery – laser tonsil cryptolysis, that is the destruction of the pathological areas in the tonsils and maintenance of the normal areas with a well-known immune role. It is worth mentioning that the therapeutic solution can be achieved by performing 3 – 4 laser interventions per year and the intervention lasts approximately 10 – 15 minutes, without intra- or post-operative pain and does not require hospital admission or cessation of the professional activity. Nevertheless, complicated forms of chronic tonsillitis (focal disease with heart, kidney or joint impairment) do not have therapeutic benefits from the laser surgery, in this case tonsillectomy is indicated.

Laser surgery is the most widely used procedure in the nose pathology, especially in nasal obstruction. Most of the time patients who complain they are not able to breathe through their nose show either a nasal septum deviation or a form of rhinitis (hypertrophic chronic rhinitis, vasomotor rhinitis or allergic rhinitis).

Only some nasal septum deviations (unique septal crest, without an antero-posterior trajectory) can benefit from laser surgery.

The nasal cornets are endonasal structures found on the lateral parts of the nasal cavities. The rhinitis forms previously mentioned are accompanied by increased cornets, decreased respiratory space and a sensation of nasal obstruction.

Laser vaporization of nasal cornets (with or without endoscopic control) determine a decrease of their volume, subsequently ensuring suitable breathing. It is an intervention that is performed on the consultation chair, with local contact anesthesia (not by injection), which lasts approximately 10 minutes for each cavity and which does not require nasal bandage. The patient can go home after approximately 45 minutes with suitable breathing and he/she needs to return for a nose check-up in the following 24 – 72 hours without having to interrupt his/her usual activitie

Laser surgery, so widely spread in ENT clinics and private practice in the whole world, ensures the requirements of each patient: short, pain-free surgical interventions, with local anesthesia, without hospital admission and without impairing the day-to-day social and family activities.
